Please bring your insurance card and be sure to check with your insurance to see if you need a referral first.  Many patients will change their insurance at the beginning of the year so be sure you check to see if you have a new insurance card before coming in for your appointment.

Most insurance plans will have a annual deductible, which will have to be paid along with your co-pay before the insurance starts paying any portion of the bill.  Usually this is only a problem at the beginning of the year.  Again, you will have to check your policy to see how much your individual deductible and co-pays might be.

If you are a new patient please fill out our New Patient Forms and bring them with you to your first appointment.
